Nevada Birth Parent Application: Jurat for the Adoption Reunion Registry

The Nevada Birth Parent Application allows a birth parent to register voluntarily with the Nevada Adoption Reunion Registry and express interest in contact with an adult child who was adopted. DCFS describes the Registry as a mutual-consent process, so a family connection cannot occur unless the adult adoptee also submits a qualifying Registry application.

The form is not a consent to a new adoption, does not terminate parental rights, and does not automatically open sealed adoption records. It registers the birth parent's contact and adoption information for the Registry's matching process concerning the child identified on the application.

The certificate says “subscribed and sworn to,” and the form requires the birth parent to sign in the notary's presence. What the Birth Parent Application Does

The application voluntarily adds the birth parent's information to the Registry for a possible match concerning the adopted child identified on the form.

  • Registers the birth parent's contact information

    The applicant supplies current names, contact information, address, and other identifying details used by the Registry.

  • Identifies the adopted child

    The form requests the child's birth name, birth date, place of birth, and available adoption information so DCFS can evaluate a potential connection.

  • Expresses interest in contact

    The birth parent states an interest in making contact with the child who was adopted. Registration does not guarantee that the adult adoptee has applied or wishes to connect.

  • Requires a matching adult-adoptee application

    DCFS states that a match cannot occur unless both applicants have notarized Registry applications on file. The adoptee cannot register until reaching 18 years of age.

  • Applies only to the applicant and named child

    The form states that the application is only for the birth parent signing it and the child identified on that application. Contact DCFS about separate registrations involving another child.

  • May be withdrawn in writing

    The applicant may withdraw from the Registry by submitting the official change or withdrawal form in writing.

The Birth Parent Registry Application Is Different From Other Adoption Requests

Several official DCFS forms concern adoption records and family information. They do not serve the same purpose.

  • Birth Parent Application

    Use this application when a birth parent wants to register for a possible match and contact with the adult adoptee.

  • Adoptee Application

    The adult adopted person uses a separate application. The birth parent cannot complete or sign the adoptee's registration.

  • Relative Part One and Part Two

    A qualifying relative uses Part One. Part Two is a separate birth-parent consent used only when DCFS coordinates that relative-registration process.

  • Request for Non-Identifying Information

    This request concerns available non-identifying adoption-file information. DCFS states that adoptees do not need birth-parent consent or Registry enrollment for that separate request.

  • Birth Parent Request for Documents Executed

    A birth parent seeking copies of adoption documents previously signed uses a different official request.

  • Petition to unseal adoption records

    Court adoption records are confidential. Opening the court file requires a separate court process and is not accomplished through this Registry application.

Sworn Execution

The Birth Parent Application Requires a Jurat

The form says the birth-parent signature must be made in the presence of a notary and uses “subscribed and sworn to” certificate wording.

  • Personal appearance is required

    The birth parent registering must personally appear. An adoptee, relative, spouse, attorney, courier, or Registry representative cannot appear for an absent applicant.

  • An oath or affirmation is required

    The applicant must swear or affirm that the statements in the application are true.

  • The applicant signs during the appointment

    Leave the birth-parent signature blank until the notary administers the oath or affirmation and witnesses the signing. This is not an acknowledgment of a prior signature.

  • Identity must be established

    Lake Mead Mobile Notary requires acceptable original, current physical identification for an in-person appointment. A photo, screenshot, scan, or photocopy is not accepted.

  • The notary completes the certificate

    The notary completes the venue, date, applicant's printed name, notary signature, seal, and other required certificate information.

  • The notary does not verify the adoption facts

    The notary does not establish biological parentage, confirm the adoption agency or adopted name, validate the applicant's Registry eligibility, or certify that the adoptee has registered.

Applicant and Signer

The Birth Parent Registering Signs the Application

The application is personal to the birth parent and the adopted child named in the form.

  • Birth mother or birth father

    The eligible birth parent registering with DCFS completes and signs the application under oath or affirmation.

  • The other birth parent's information may be listed

    The application asks for the other birth parent's name and information when known, but listing that information does not make the other parent a signer on this applicant's form.

  • The adult adoptee uses a separate application

    The adopted person cannot be registered by the birth parent and must voluntarily complete the Adoptee Application after reaching the required age.

  • A relative does not sign for the birth parent

    Relatives use the separate two-part relative-registration process. They cannot substitute their signature on this personal birth-parent application.

  • Do not assume an agent may sign

    The form requests the birth parent's personal interest, certification, and signature. Contact DCFS before booking when a power of attorney, guardianship, or another representative arrangement is proposed.

  • Eligibility remains a DCFS decision

    DCFS states that birth parents who relinquished a child for adoption or whose parental rights were terminated by a Nevada court may register when the adoption was finalized in Nevada. The notary does not adjudicate those conditions.

What to Complete and Bring to the Notary Appointment

  • Birth parent's current and former names

    Complete the requested last, first, middle, maiden, and other names used, together with date of birth and gender information.

  • Current contact and address information

    Enter the telephone numbers, email or other contact information, inmate number when applicable, home address, and mailing address when different.

  • Other birth parent's information when known

    Provide the requested names, contact details, and mailing information to the extent known. Direct uncertainty about required entries to the Registry.

  • Child's birth information

    Complete the child's birth name, nickname or other names used, date of birth, city and state of birth, and gender as requested.

  • Available adoption information

    Enter the adoption agency, child's adopted name, and adoptive parent names when available and as directed by DCFS.

  • Leave the signature and jurat incomplete

    Do not sign the birth-parent signature line or complete the notarial venue, date, applicant name, notary signature, or seal before the appointment.

  • Original, current physical identification

    Bring an identification document acceptable to Lake Mead Mobile Notary. Digital images and photocopies are not accepted.

After Notarization

Submit the Application and Keep the Registry Information Current

Follow the current DCFS instructions for submission, changes, and withdrawal rather than sending the application to a court or county clerk.

  1. Check the completed jurat

    Confirm that the signature, venue, date, printed applicant name, notary signature, and seal are visible and that the full application is included.

  2. Email a clear PDF when using electronic submission

    DCFS currently accepts notarized Registry applications by email as PDF files and may refuse submissions that are not notarized, legible, printable, or in PDF format.

  3. Verify the current Registry email

    The official Registry page currently directs emailed applications to [email protected]. Confirm the address before sending sensitive information.

  4. Use the application address when mailing

  5. Keep contact information current

    The birth parent is responsible for notifying the Registry of name, address, telephone, email, medical, or other registration changes through the official change form.

    A Registry application may be withdrawn by submitting the official Request for Change of Address or Other Registration Changes and selecting the withdrawal option.

Does the Birth Parent Application require notarization?

Can the birth parent sign before the notary arrives?
