The VP-241 beneficiary must submit several required documents with the form: a certified copy of the deceased owner's death certificate (not a photocopy), any available identification documents from the deceased, proof of insurance in the beneficiary's name, and the completed VP-241 affidavit. Supporting documents like the vehicle registration or bill of sale may also be requested by Nevada DMV.

The VP-241 has strict signature requirements: it must be signed in original ink in the presence of either a Nevada notary public or an authorized DMV representative—electronic signatures or pre-signed forms are not accepted. Once the form is signed and witnessed, no alterations, corrections, or changes can be made. Any modifications after notarization will result in rejection, requiring a completely new form to be prepared and notarized.
