File VP-202 when your Nevada plates are lost, stolen, mutilated/illegible, destroyed, left on a sold vehicle, or surrendered/confiscated. For stolen plates, include the law-enforcement case number. Nevada DMV notes that stolen plates cannot be duplicated and you must visit a full-service DMV office for new plates.
