You have three options: (1) Notarize VP-012 and mail the original to DMV; (2) take the unsigned form to DMV and sign in front of a DMV representative; or (3) start online with Turbo Titles to pre-stage the request, then finish in person with required originals and fees. Many customers choose mobile notarization so their packet is “DMV-ready” before they arrive.
