What to know about this form

EC-008 is a sworn affidavit. The form contains a perjury statement and a jurat to be completed by a Notary Public or an Authorized DMV Representative. If you file by mail, you must have it notarized because you’re not appearing before DMV staff. If you visit a DMV office, you may sign there before an authorized representative instead of a notary.
